Iron Tracer Ultrasound-Guided Core Biopsy in Identifying Sentinel Lymph Nodes before Surgery in Patients with High Risk Ductal Breast Carcinoma in Situ or Hormone Receptor Positive Stage I-II Breast Cancer

This clinical trial studies the side effects of an iron tracer ultrasound-guided core biopsy and how well it works in identifying sentinel lymph nodes before surgery in patients with high risk ductal breast carcinoma in situ or hormone receptor positive stage I-II breast cancer. Diagnostic procedures, such as an iron tracer ultrasound-guided core biopsy, may be a less invasive way to check for breast cancer and find out how far the disease has spread.

Inclusion Criteria

High risk ductal carcinoma in situ (DCIS) or invasive stage I and II estrogen receptor (ER) positive (+)/progesterone receptor (PR)+ breast cancer with negative clinical lymph node exam
Receiving care in the Breast Surgery Department at Columbia

Exclusion Criteria

Receiving neoadjuvant chemotherapy
Have a recurrence of ipsilateral breast cancer
Have had a previous ipsilateral axillary dissection or lymph node biopsy

PRIMARY OBJECTIVES:

I. To evaluate the feasibility of a pre-operative sentinel lymph node core biopsy utilizing iron-tracer guided ultrasonography in hopes to eventually substitute for traditional surgical excisional sentinel lymph node biopsy techniques in patients with early stage breast cancer and in situ disease.

OUTLINE:

Patients receive iron sucrose injection intradermally (ID) and then undergo sentinel lymph node ultrasound-guided core biopsy. Patients then undergo standard of care axillary lymph node dissection (ALND) or sentinel lymph node biopsy (SLNB) no greater than 30 days after sentinel lymph node ultrasound-guided core biopsy.
